We accept all auto and workers’ compensation plans but the patient must provide information including the claim number and adjuster’s contact information before the appointment.

 

Healthshare plans are treated like self or cash pay. The patient pays the time of service fee upfront and we’ll provide a receipt for them to send for reimbursement.
